What should you know about neurologic thoracic outlet syndrome?

Short answer
Neurologic thoracic outlet syndrome occurs when nerves passing from the neck toward the arm are irritated or compressed near the thoracic outlet, causing symptoms such as pain, tingling, numbness, or weakness in the arm or hand.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main structure involved
- Nerves travelling from the neck to the arm
- Common symptom pattern
- Pain, tingling, numbness, or weakness in an arm or hand
- Common first approach
- Assessment, activity modification, and guided rehabilitation
What it means
The thoracic outlet is a narrow passage between the lower neck, collarbone, and upper ribs. It contains nerves and blood vessels travelling toward the arm. In the neurologic form, pressure or irritation affects part of the brachial plexus, the network that carries movement and sensation signals to the arm and hand.
Symptoms may be felt on one side and can extend from the neck or shoulder into the forearm, hand, or fingers. They may become more noticeable when the arm is raised, when the shoulders are held forward, or during repeated overhead activity. A person may describe burning, aching, pins and needles, reduced grip, hand clumsiness, or a tired feeling in the arm. These symptoms can overlap with neck nerve irritation, carpal tunnel syndrome, or other conditions, so the pattern and examination matter.
What you can do
Arrange an assessment if symptoms keep returning, interfere with sleep or work, or are accompanied by weakness. A clinician may ask about activities, posture, previous injury, and the positions that bring symptoms on. They may check neck and shoulder movement, sensation, strength, reflexes, and circulation, then decide whether tests or imaging are appropriate.
Treatment commonly starts with activity adjustment and a guided rehabilitation plan. Physiotherapy may address shoulder-blade control, neck and upper-chest mobility, breathing mechanics, and gradual nerve-friendly strengthening. Postural correction therapy can help you practise less compressed positions during sitting, lifting, and computer work. Avoid forcing stretches or repeatedly testing the position that triggers tingling. Keep a brief record of the activity, arm position, duration, and symptoms to discuss at the appointment.
When to seek care
Seek urgent medical care if arm or hand symptoms begin suddenly with facial drooping, speech difficulty, severe chest pain, fainting, or new trouble walking. Prompt assessment is also needed for rapidly worsening weakness, a hand that becomes cold or changes colour, marked swelling, or severe pain after an injury.
For gradually developing numbness or tingling without these warning signs, book a routine appointment. Earlier review is sensible when you are dropping objects, losing grip, or noticing that symptoms are spreading or becoming more frequent.
Questions for your doctor
Ask which findings support nerve compression at the thoracic outlet and which alternative causes should be considered. You can also ask which activities to modify, whether physiotherapy is suitable for your strength and symptom pattern, how progress will be assessed, and which changes should prompt an earlier review.

Questions people ask
Can neurologic thoracic outlet syndrome affect the hand?
Yes. Irritated nerves can cause tingling, numbness, aching, reduced grip, or clumsiness in the hand and fingers, sometimes together with symptoms in the shoulder or arm.
Does a scan confirm neurologic thoracic outlet syndrome?
A scan may help investigate other causes or show relevant anatomy, but diagnosis usually depends on the symptom pattern, physical examination, and the results of any tests your clinician considers appropriate.
What should you avoid while waiting for an assessment?
Avoid repeatedly holding the arm in the position that provokes symptoms, forceful stretching, and heavy or prolonged overhead activity until you have individual guidance.
