What should you know about neurogenic fever?

The brain helps regulate heat production and heat loss. Injury or disease affecting temperature-control pathways can disturb this process. A temperature reading alone cannot establish the cause: infections such as acute bronchitis, cellulitis, COVID-19 or other illnesses can also produce fever.

A clinician may review recent illness, medicines, neurological symptoms and the pattern of temperature changes. Examination and selected tests can look for infection or another treatable trigger. Do not assume a fever is neurogenic because a neurological condition is already known.

Record the temperature, time, measurement method and associated symptoms. Drink fluids if you can do so safely, rest in a cool environment and avoid heavy clothing. Use fever medicine only according to the label or a clinician’s advice, especially if you have liver, kidney or stomach problems or take other medicines.

Arrange medical advice promptly when fever is unexplained, persistent, or follows a head injury, stroke, seizure or other neurological event. Bring a list of medicines and describe chills, cough, pain, rash, confusion or changes in movement.

Seek emergency help for fever with confusion, unusual sleepiness, a seizure, severe headache, stiff neck, trouble breathing, chest pain, a new weakness, or difficulty speaking. Get urgent assessment after significant head injury or if the person cannot drink, keeps vomiting, or is becoming difficult to wake.

Could infection or a medicine explain this temperature? Does the neurological history change the tests needed? Which temperature readings or symptoms should prompt emergency care? How should fluids, cooling and fever medicine be managed?
