What should you know about neuroblastoma black eye?

Neuroblastoma is a cancer of immature nerve cells. When it spreads to or presses on tissues behind the eye, small blood vessels and surrounding structures can be affected. This may produce dark colour around one or both eyes, swelling, or the appearance of a black eye without a clear injury. A child may also have pain, changes in eye movement or vision. A bruise after a bump is far more common, so the appearance alone does not establish neuroblastoma.

A clinician will ask about injury, timing, pain, fever and other symptoms, then examine the eyes and general health. If the pattern is unexplained, persistent, associated with swelling, or accompanied by other concerning findings, imaging and specialist tests may be arranged.

Arrange a medical assessment for unexplained bruising around a child’s eye, especially if it is not improving or appears on both sides. Do not press on a swollen eye. Note when the colour appeared, any injury, changes in vision, fever, pain or unusual tiredness. Seek urgent care for sudden vision loss, severe eye pain, inability to move the eye normally, rapidly increasing swelling, repeated vomiting, severe headache, confusion or unusual difficulty waking. After a head injury, emergency assessment is needed for loss of consciousness, seizure or worsening symptoms.

Ask whether the appearance fits an injury or needs further testing, what examination findings are concerning, and whether imaging is recommended. If neuroblastoma is being considered, ask whether the eye is affected by pressure or spread, which tests clarify this, and how vision will be monitored.
