What causes near fainting?

Near fainting can result from a temporary drop in blood pressure, dehydration, low blood sugar, blood loss, medication effects, or a heart rhythm problem. The medical term is presyncope: you may feel lightheaded, weak, sweaty, nauseated, or as though your vision is narrowing, while remaining conscious. The trigger may be simple, such as standing quickly, overheating, prolonged standing, pain, or emotional distress, but a first episode, repeated episodes, or symptoms during exertion deserve assessment.

What near fainting may mean

Near fainting often reflects reduced circulation to the brain for a short time. Dehydration can reduce circulating fluid, while standing can allow blood to pool in the legs. Anaemia may present with dizziness, tiredness, breathlessness, or a fast heartbeat because the blood carries less oxygen. Hypoglycaemia can cause sweating, shaking, hunger, confusion, and weakness. Atrial fibrillation and other rhythm disturbances may reduce the heart's efficiency or produce an irregular pulse. These are possibilities, not a diagnosis; the context and examination matter.

What to do during an episode

Stop what you are doing and lie flat with your legs raised if you can do so safely. If lying down is not possible, sit and lower your head between your knees. Loosen tight clothing, move away from heat, and take slow breaths. Do not drive, climb, or continue exercising until you feel fully recovered. Once alert and able to swallow, drink water; if diabetes or glucose-lowering treatment is relevant, follow your personal low-blood-sugar plan. Record what you were doing, your position, food and fluid intake, medicines, warning symptoms, and how long recovery took.

When to seek care

Seek emergency help if near fainting occurs with chest pain, severe breathlessness, a new irregular or very fast heartbeat, sudden weakness, trouble speaking, seizure-like movements, severe headache, heavy bleeding, or persistent confusion. Get urgent assessment after an episode during exercise, while lying down, or after an injury. Arrange a medical appointment for a first unexplained episode, repeated symptoms, pregnancy, known heart disease, possible anaemia, or medicines that can lower blood pressure. A clinician may check your blood pressure lying and standing, pulse, heart tracing, blood count, glucose, and hydration status.

Ask what causes best fit the timing and warning symptoms, whether any medicine or supplement could contribute, and whether you need an electrocardiogram or blood tests. Bring your episode notes and a complete medicine list. Mention palpitations, dark stools, heavy menstrual bleeding, reduced fluid intake, vomiting, diarrhoea, diabetes, and family history of sudden cardiac death, even if the symptoms were brief.
