Natural childbirth with twins

The maternity team considers each baby’s position, growth, placentae, pregnancy complications and your health. A Growth Scan helps track growth and fluid, while ultrasound near labour may clarify presentation. If the first baby is head-down and other findings are reassuring, vaginal birth may be discussed. A Caesarean Section may be recommended when positions, growth or wellbeing make labour less suitable.

Twin labour often involves close monitoring and a team prepared for changes after the first baby is born. The second baby can change position, so the plan is reviewed throughout labour.

Attend growth and antenatal appointments, learn when to call, and discuss pain relief, monitoring, birth companions and feeding two babies. Ask where the birth will take place and which specialists may be present. Keep transport and support arrangements ready because the recommended plan can change with labour findings or either baby’s condition.

Contact your maternity unit urgently for vaginal bleeding, regular contractions, leaking fluid, severe abdominal pain, fever or reduced movement from either baby. Seek emergency help for heavy bleeding, faintness, breathing difficulty or severe pain. Do not wait for the next scan if something feels suddenly different.

Ask: Which baby is leading and where are both babies positioned? Are their growth patterns reassuring? What monitoring will I have in labour? What would make a Caesarean Section safer? Who will be present for the birth and early newborn checks?
