What should you know about nappy rash that bleeds?

Even a small amount of blood can come from cracks, rubbing, or skin that has been rubbed raw. Avoid scrubbing, picking, or trying to remove stuck cream from the sore area.

Nappy rash commonly develops where moisture, urine, stool, warmth, and friction weaken the skin barrier. Once the surface cracks, wiping can reopen it and cause spotting. Bright-red, raw skin in the folds or around the anus may also occur with a yeast-associated rash, especially when the rash continues despite ordinary barrier care. Bleeding does not identify one cause by itself.

Look at the pattern rather than the blood alone. Note whether the sore skin is mainly on the areas touching the nappy, whether the folds are involved, and whether there are separate spots, blisters, crusts, swelling, or discharge. Change the nappy promptly after wetting or soiling. Rinse with lukewarm water or use a fragrance-free wipe, then pat rather than rub. Allow the skin to dry briefly before applying a thick protective barrier recommended for babies. Use a fresh nappy that does not press tightly on the damaged area, and give short periods without a nappy when practical.

Do not use antiseptics, fragranced products, talc, or leftover steroid or antifungal creams unless a healthcare professional has advised them for this child. If stool is stuck to the skin, soften it with water instead of pulling. Wash your hands before and after care, and keep track of how often the skin bleeds and whether the rash is spreading.

Arrange prompt medical assessment when bleeding recurs, does not stop with gentle pressure, or appears alongside increasing pain, marked swelling, pus, yellow crusting, blisters, open ulcers, or rapidly spreading redness. Seek urgent help if the baby seems unusually sleepy, difficult to settle, feverish, is feeding poorly, or has fewer wet nappies than expected.

A healthcare professional should also review a rash that is not improving after careful nappy changes and barrier protection, or one that repeatedly returns. Take a clear description of products used and any recent diarrhoea, antibiotics, or skin problems.

Ask: Could the bleeding be from cracked irritation, yeast, or another skin problem? Which cleanser and barrier product should we use? Should any treatment be applied to the folds? What changes mean we should return quickly?
