What should you know about NAC and heart palpitations?

Palpitations can feel like pounding, fluttering, skipped beats, or a heartbeat that suddenly speeds up. They may follow stress, poor sleep, caffeine, nicotine, fever, dehydration, or some medicines and supplements. Other health problems, including anaemia, thyroid overactivity, low blood sugar, or an abnormal rhythm such as atrial fibrillation, can also cause them.

A temporal link with NAC does not prove that NAC caused the symptom. Note whether the sensation began after a dose, how long it lasted, whether the rhythm felt regular, and what you were doing. A clinician may check your pulse, blood pressure, heart rhythm, blood tests, or an ECG depending on the story.

Pause self-directed supplements and contact the prescriber or pharmacist if palpitations started after NAC, particularly if you take heart, thyroid, diabetes, or blood-pressure medicines. Do not stop essential prescribed medicines without advice. Keep hydrated unless you have been told to restrict fluids, and avoid excess caffeine, nicotine, and stimulant products while the symptom is being clarified.

Keep a brief record of timing, duration, pulse if you can measure it safely, triggers, associated symptoms, and all medicines or supplements. Bring the product label to an appointment. Assessment may be especially useful if episodes recur, occur during exertion, or are accompanied by dizziness.

Call emergency services for palpitations with chest pain or pressure, severe breathlessness, fainting, new weakness, confusion, or blue or grey lips. Seek same-day medical advice for a sustained fast or irregular heartbeat, repeated episodes with dizziness, or symptoms during exercise. Do not drive yourself if you feel faint or severely unwell.
