A local ache after a demanding lift may reflect muscle strain or non-specific low back pain. Pain made worse by arching or twisting can involve the small facet joints; pain that travels down a leg with tingling or weakness can occur with a herniated disc or nerve irritation. The sacroiliac joint can produce pain near one side of the pelvis. Myofascial pain syndrome may cause tender bands in surrounding muscles. Lumbar spondylosis and spinal stenosis are additional possibilities, especially when symptoms predate the lift. None can be confirmed from the lifting event alone.

What to do now

Pause heavy lifting, repeated bending, and twisting while the pain is irritable. Continue easy walking and ordinary movement as tolerated, changing position rather than staying in bed. Heat or a wrapped cold pack may provide comfort. Before returning to deadlifts, have a physiotherapist review hip mobility, bracing, bar path, load, and fatigue. Build back gradually and stop if symptoms spread or strength changes.

Get prompt assessment for progressive weakness, significant numbness, difficulty walking, or pain shooting into the leg with worsening symptoms. Emergency care is needed for new bladder or bowel problems, numbness around the saddle area, or rapidly developing weakness. Seek urgent advice when fever, major trauma, or unexplained illness accompanies the back pain.
