My ankle hurts but has no swelling: what could it mean?

Short answer

An ankle can hurt without swelling because irritation may involve a tendon, joint, ligament, bone, or nerve before visible inflammation appears.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Key point
No swelling does not rule out an ankle injury.
Track
Note triggers, walking difficulty, warmth, redness, and duration.

What it may mean

Pain after a twist can reflect an ankle sprain even when the ankle looks normal. Repeated loading, footwear changes, or a sudden increase in walking can irritate tissues around the joint. Stiffness that is worse after rest may fit ankle osteoarthritis, while intense warmth, redness, and sudden tenderness can occur with gout. A small fracture is also possible after an impact or awkward landing, particularly when weight-bearing remains difficult. The absence of swelling does not identify the cause or rule out an injury.

What to do now

For a mild, recent ache, reduce running, jumping, and uneven-ground walking for a short period. Keep gentle ankle movement within comfort, use supportive shoes, and elevate the leg if it feels heavy. Cold or warmth may help different people; protect your skin and stop if symptoms worsen. Do not force stretches or massage an area that is sharply tender. Arrange an assessment if pain is not settling, returns with activity, or changes how you walk. A clinician can examine movement and decide whether imaging or rehabilitation is appropriate.

When to seek care

Seek urgent medical care after a significant injury if you cannot take steps, the ankle looks deformed, or pain is severe. Get prompt help for a hot, red ankle with fever, rapidly increasing symptoms, numbness, unusual weakness, or a foot that becomes pale or cold. Calf swelling or breathlessness alongside leg symptoms needs urgent attention. If none of these warning signs apply, book a routine review for persistent or recurring pain.

Questions for your doctor

Could the pain be coming from a tendon, joint, ligament, bone, or nerve? Do I need an examination or imaging? Which activities should I pause, and which movements are useful? What symptoms would mean I should return sooner?

Questions people ask

Can an ankle fracture have little swelling?

Yes. A fracture can sometimes present mainly with focal tenderness and difficulty bearing weight, so an examination is useful after an impact or awkward landing.

Should I keep exercising?

Avoid movements that increase pain, but gentle comfortable motion can help prevent stiffness while you arrange advice if symptoms persist.
