Understanding the injury
A twist can stretch or tear ankle ligaments, causing pain, bruising, and swelling that may increase during the first part of recovery. An Ankle Sprain is one possible explanation. A crack in the bone can occur with the same type of incident, particularly when there is marked tenderness directly over an ankle bone, an unusual shape, or difficulty taking steps. Ankle Osteoarthritis or Gout can cause ankle symptoms too, but they are less directly tied to a single twisting event. A clinician uses the injury details and examination to decide whether imaging is needed.
First steps
Stop the activity and protect the ankle from further twisting. Rest it in a comfortable position, raise it when possible, and use a cold pack wrapped in cloth for short periods. A light support may help if it does not cause numbness, colour change, or extra pain. Use a stable shoe or mobility aid if advised, and avoid running, jumping, heat, and alcohol soon after the injury. Do not try to straighten a deformed ankle. Arrange assessment if walking remains difficult or pain is focused on bone.
Warning signs
Seek urgent care for deformity, an open wound, severe or escalating pain, a foot that becomes cold, pale, or blue, new numbness, or inability to move the toes. Prompt review is also needed for rapidly increasing swelling or fever with spreading redness.
