Mirena spotting after sex: what to know

A small amount of blood after sex may follow dryness, friction, or a sensitive cervix. Mirena can also change cervical mucus and bleeding patterns, particularly around insertion or while the body is adjusting. Spotting that keeps recurring, occurs without sex, or comes with pain or discharge needs a closer look.

A clinician may assess the cervix, vagina, and uterus. Cervical infection, including [Chlamydia](/conditions/chlamydia/), cervical dysplasia, polyps, and fibroids can present with bleeding after sex. Cervical cancer is less common, but abnormal bleeding is a reason for examination, not a reason to assume a cancer diagnosis.

Make a routine appointment if spotting happens more than once, lasts beyond the next day, or is new after a period without bleeding. Record the timing, amount, pain, discharge, contraception history, and the date Mirena was fitted. A clinician may perform a pelvic examination, check for infection, review cervical screening, and arrange imaging when appropriate.

Avoid inserting anything into the vagina while bleeding is active if it increases discomfort. Do not pull on the device threads or remove Mirena yourself. Use condoms if infection is possible until you have advice and testing.

Seek urgent help for heavy bleeding, severe pelvic pain, faintness, fever, foul-smelling discharge, or a positive pregnancy test with pain or bleeding. These symptoms can signal a complication that needs prompt assessment, regardless of whether Mirena is in place.

Ask whether your cervix looks irritated, whether infection testing or cervical screening is due, and whether the device position should be checked. Ask what bleeding amount or pain should lead you to urgent care.
