What should you know about Mirena and hirsutism?

Hirsutism means coarse, dark hair growing in a pattern more commonly associated with male hormones, such as the face, chest or lower abdomen. Mirena releases levonorgestrel in the uterus. A clinician considers the timing of hair growth, menstrual pattern, family history, other medicines and signs of androgen excess before deciding whether the device is relevant.

The same symptom can occur with conditions such as polycystic ovary syndrome or other hormone disorders. Hirsutism itself does not identify one cause, and the presence of facial hair does not mean a serious condition is present.

Arrange a review if hair growth is new, progressing or distressing. Note when it began, where it appears, cycle changes, acne, scalp hair thinning and any voice or body changes. Do not remove Mirena or start hormone treatment without discussing contraception and alternatives with the clinician who manages your care.

Hair-removal choices can be matched to hair colour, skin type, area and budget. Laser hair removal may reduce growth in suitable hair, while electrolysis targets individual follicles. Spironolactone treatment may be considered for some people, with contraception and medical monitoring discussed first.

Seek prompt medical assessment if coarse hair develops quickly or comes with a deeper voice, rapidly increasing muscle bulk, severe acne, scalp hair loss or marked menstrual change. Seek urgent care for severe abdominal pain, fainting or heavy bleeding related to an intrauterine device.
