What should you know about milk thistle and tamoxifen?

Tamoxifen is changed by enzymes in the body into active forms that contribute to its effect. Milk thistle contains plant compounds and supplement products can differ in strength, ingredients and preparation. A possible interaction could change the amount of tamoxifen or its active forms, although the clinical effect cannot be predicted from the label alone.

Keep tamoxifen on the schedule prescribed unless the prescriber changes it. If you have already taken milk thistle, record when and how much you used and tell the team; do not take extra tamoxifen to compensate. Tamoxifen may be part of Hormone Therapy for Cancer or Risk-Reducing Endocrine Therapy, so the reason for treatment matters.

Seek urgent help for trouble breathing, facial or throat swelling, fainting, severe chest pain, sudden one-sided weakness, or heavy unusual bleeding. Contact your treatment team promptly about new leg swelling or pain, unexplained vaginal bleeding, vision changes, or persistent symptoms after starting a supplement. These symptoms need assessment rather than an assumption that milk thistle is the cause.
