Short answer

Mild spotting in early pregnancy can occur for several reasons, but any bleeding should be discussed with a pregnancy-care clinician because assessment depends on timing, amount, pain, and other symptoms.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Spotting
A small amount of blood, often seen when wiping or on underwear
Track
Amount, colour, timing, pain, cramps, and other symptoms
Urgent signs
Heavy bleeding, severe or one-sided pain, faintness, or shoulder-tip pain

What mild spotting can mean

Spotting means a small amount of blood, often noticed on toilet paper or underwear, rather than flow that soaks a pad. In early pregnancy, the cervix may bleed more easily, including after sex or an examination. Other possibilities include changes around the developing pregnancy, an infection, or a pregnancy complication. The appearance of blood alone cannot identify the cause.

Bleeding with one-sided or severe pelvic pain, shoulder-tip pain, faintness, or marked weakness needs urgent assessment because it can signal a complication such as an ectopic pregnancy. A clinician may ask about the timing and colour of the blood, examine you, and arrange pregnancy hormone testing or an ultrasound when appropriate.

What to do now

Contact your maternity service, obstetrician, or other pregnancy-care clinician and describe when the spotting began, how much you have seen, and whether it is continuing. Use a pad so you can observe the amount; avoid tampons. Note cramps, clots, fever, dizziness, unusual discharge, or pain during urination. Until you have advice, take it easy and avoid sex if it brings on bleeding or discomfort. Do not start or stop medicines without checking with a clinician.

When to seek urgent care

Seek emergency care now for heavy or rapidly increasing bleeding, severe abdominal or pelvic pain, pain focused on one side, shoulder-tip pain, fainting, severe dizziness, or feeling unusually weak. Get prompt medical advice for fever, chills, foul-smelling discharge, painful urination, or bleeding that persists or returns. If you know you have a negative blood type, mention this when you contact the service, as your clinician can advise whether any treatment is needed.

Questions for your clinician

Ask what symptoms should trigger emergency care, whether you need an examination or ultrasound, and whether you should change activity or avoid sex for now. Tell the clinician the first day of your last menstrual period, your pregnancy test result, any previous ectopic pregnancy or miscarriage, fertility treatment, blood type if known, and medicines you take. These details help guide the next step.
