What it means

Most children with a middle ear infection notice ear pain or pulling at the ear, and some have fever, poor sleep, reduced appetite, or irritability. A child may not describe pain and instead seem less responsive to sounds. Congestion from a cold can block the tube that drains the middle ear, allowing fluid to build up behind the eardrum. A clinician confirms the diagnosis by looking at the eardrum; symptoms alone cannot show whether antibiotics are needed.

What parents can do

Offer fluids and rest, and use child-safe pain relief only as directed for the child’s age and weight. Do not put cotton buds, oil, or unprescribed drops into the ear. Keep follow-up advice, particularly if hearing remains reduced or symptoms return. A clinician may recommend observation, treatment for pain, or antibiotics depending on the child’s age, examination, severity, and overall health.

When to seek care

Arrange prompt assessment if the child is very unwell, has swelling or redness behind the ear, severe headache, neck stiffness, repeated vomiting, new facial weakness, or fluid draining from the ear. Seek urgent help for breathing difficulty, unusual drowsiness, or a child who is difficult to wake. Persistent pain or fever also deserves medical review.
