What should you know about metastatic bone prostate cancer treatment?

Bone metastases are deposits of prostate cancer in bone, not a separate type of bone cancer. They may cause aching or sharp pain, reduced mobility, weakness, or fractures, although some people have no bone symptoms at first. Doctors use symptoms, examination, blood tests, and imaging to understand which bones are involved and whether the spine or weight-bearing bones need special protection.

Difficulty passing urine can occur with prostate problems, but it does not by itself show that cancer has spread to bone. A clinician may assess urinary symptoms separately while planning cancer care.

A treatment plan may include medicines that suppress prostate cancer signals, chemotherapy, or other systemic treatment. Radiotherapy can target a painful or vulnerable area, while targeted radioisotope therapy may be considered when the cancer has a suitable marker and other criteria are met. Bone-strengthening therapy can reduce skeletal complications for selected patients; dental review and monitoring may be needed before and during it.

Surgery is not usually the only treatment for metastatic disease, but an operation may help stabilize a threatened fracture or relieve pressure in selected circumstances. Brachytherapy and prostate removal surgery have specific roles in prostate cancer and are not automatically suitable when disease has spread to bone. Gentle activity, fall prevention, pain management, and calcium or vitamin advice should be individualized by the care team.

Get emergency help for new leg weakness, numbness, difficulty walking, loss of bladder or bowel control, or severe new back pain, as these can signal spinal-cord pressure. Seek urgent review after a fall with sudden bone pain, inability to bear weight, or a limb that looks deformed. Report uncontrolled pain, new confusion, or difficulty passing urine promptly.
