What spotting may mean

Menopause is reached after twelve months without a menstrual period. During the transition before then, changing hormone levels can make bleeding lighter, heavier, closer together, or less predictable. Spotting can also come from vaginal or cervical dryness, irritation, polyps, infection, medicines, or changes in the womb lining. Pelvic organ prolapse may cause pressure, rubbing, or associated bleeding and needs an examination to identify the source.

Bleeding after menopause is not automatically cancer, but it is not something to diagnose at home. A clinician may ask about the colour, amount, timing, pain, sexual activity, medicines, and whether the blood could have come from the urine or bowel.

What to do about spotting

Arrange a medical appointment and keep a simple record of when the spotting appeared, how long it lasted, how much protection you needed, and whether you had cramps, pelvic pressure, discharge, dizziness, or bleeding after sex. Mention any hormone treatment, blood-thinning medicine, contraception, or recent procedure. Do not put products into the vagina to stop bleeding unless a clinician has recommended them.

If dryness or discomfort is present, tell the clinician rather than assuming it explains the bleeding. They can decide whether an examination, pregnancy test when relevant, swab, ultrasound, or another test is appropriate.

When to seek urgent care

Seek urgent medical help if bleeding is heavy or rapidly increasing, you feel faint or weak, have severe or worsening pelvic pain, develop fever, or notice bleeding with a possible pregnancy. Seek prompt assessment for any bleeding after twelve months without a period, even when it is only a small amount or happens once.
