Menopause and spotting after intercourse

Lower oestrogen during and after menopause can make vaginal tissue thinner, drier, and more easily irritated. Friction may then cause a small amount of blood. Infection, a cervical polyp, changes on the cervix, pelvic organ prolapse, or a uterine problem can also cause bleeding after intercourse. The timing after sex is useful information, but it does not identify the cause by itself.

Pause if intercourse causes pain or bleeding, and write down when it happened, how much blood there was, and whether you noticed dryness, discharge, or pelvic pain. A water-based lubricant may reduce friction; a clinician can discuss moisturisers or other treatments if dryness continues. Arrange an appointment for first-time, recurrent, or unexplained bleeding, and mention contraception, hormone therapy, and medicines that affect bleeding.

Seek urgent care for heavy bleeding, severe pelvic or abdominal pain, faintness, fever, or feeling suddenly unwell. Contact a clinician promptly if bleeding occurs after periods have stopped, keeps returning, or is accompanied by unusual discharge or a new pelvic pressure. Assessment may involve examining the vagina and cervix and deciding whether further tests are needed.
