Menopausal spotting when wiping: what to know

A small mark on toilet paper can arise from the vagina, cervix, womb or nearby skin and urinary or bowel tissue. After menopause, reduced oestrogen may leave vaginal tissue dry and delicate, making it prone to irritation. Polyps, fibroids and infection can also cause unexpected bleeding. Cervical dysplasia and cervical cancer can present with bleeding, while womb cancer can present with bleeding after menopause; spotting cannot tell these causes apart.

Notice whether the blood is bright or dark, whether it follows sex or straining, and whether it appears with pain, discharge, urinary symptoms or a visible skin crack. These details help a clinician decide which examination or test is appropriate.

Arrange a medical appointment and write down the date, amount, colour and possible source of the spotting. Mention hormone medicines, blood thinners, recent sex, pelvic procedures and whether you have had similar episodes. Use gentle washing and avoid inserting products until advised. Do not assume a tiny amount is harmless or wait for it to become heavy before seeking advice.

Contact a clinician promptly for any bleeding after menopause, especially if it recurs. Seek urgent help for heavier bleeding, clots with weakness, fainting, severe pelvic pain, fever or dizziness. Bleeding with a foul discharge, unexplained weight change or difficulty passing urine also deserves prompt review. Emergency care is appropriate if you feel unstable or cannot manage the bleeding.
