Membrane sweep: what to expect

A membrane sweep may help labour begin, but no single figure predicts what will happen for every pregnant person. The response depends partly on how ready the cervix is, how far along the pregnancy is and whether the body is already showing signs that labour may start soon. A sweep can be helpful without leading to labour immediately, and some people may need another discussion about next steps.

During a membrane sweep, a clinician gently separates the lower part of the amniotic sac from the cervix using a finger during an examination. This can encourage the release of local hormones involved in cervical change and contractions. The examination may be uncomfortable, with cramping or pressure during and afterwards. Light bleeding or a blood-streaked mucus discharge can occur. These effects do not by themselves confirm that active labour has started.

A sweep is more likely to be considered when the cervix can be reached and pregnancy timing makes it appropriate. If the cervix is closed, high or difficult to reach, the sweep may not be possible or may have less immediate effect. A sweep does not break the waters, and it is different from medical induction, which may use medicines or other procedures to start labour.

Follow the advice from your maternity team about normal activity, pain relief and when to contact them. You can use a pad to observe bleeding or fluid and note the timing and pattern of cramps. Rest, drink fluids and keep your maternity contact details available. If labour does not begin, your clinician can explain whether another sweep, waiting or a planned induction fits your pregnancy and examination findings.

Before agreeing to a sweep, ask what the examination may feel like, why it is being offered now and what alternatives are available. You can discuss the procedure and make an informed choice; consent should be obtained before the examination.

Contact your maternity unit promptly for heavy bleeding, severe or worsening pain, reduced or changed fetal movements, or fluid leaking from the vagina. Seek urgent help if you feel very unwell, faint or short of breath. Call your maternity team if contractions become regular and strong, or if you are unsure whether your waters have broken. Use the emergency service recommended where you live when symptoms feel immediately dangerous.

Ask: How ready is my cervix for a sweep? What changes should I expect after the examination? Which bleeding, pain or fluid symptoms should prompt a call? If labour does not start, when will we review the plan and discuss induction?
