Short answer

Melancholic depression is a depressive pattern with marked loss of pleasure and noticeable changes in mood, sleep, appetite, energy, or movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What melancholic depression can look like

People may lose interest even in activities they normally enjoy, rather than simply feeling sad about a particular event. Sleep may become disturbed, with early waking or difficulty sleeping, and appetite can change. Some people feel physically slowed, speak or move less, and struggle to begin ordinary tasks; others feel unusually restless or agitated. Fatigue and difficulty concentrating can make work, study, and decisions harder. These features can occur within major depression and need an assessment because other physical or mental health conditions can produce similar changes.

Steps that can help

Arrange a GP or mental health assessment, especially when symptoms persist, interfere with daily life, or are worsening. Tell the clinician when the changes began, how sleep and appetite have shifted, and whether you have had previous episodes or periods of unusually high energy. Treatment may include talking therapy, an antidepressant, or both, chosen around your symptoms and medical history. Keep a simple record of sleep, meals, mood, and energy. Maintain regular meals, daylight exposure, gentle activity, and contact with a trusted person while awaiting care. Do not stop prescribed medicine suddenly without advice.

When to get urgent help

Seek urgent help now if you are thinking about suicide, self-harm, or harming someone else, or if you cannot keep yourself safe. Contact emergency services or go to an emergency department if severe confusion, extreme agitation, or inability to eat and drink develops. If symptoms are steadily worsening, contact a clinician promptly. Depression can also involve insomnia, daytime sleepiness, fatigue, irritability, or feeling anxious, so mention these changes rather than treating each one in isolation.

Questions for your doctor

Could this pattern be melancholic depression or another condition? What treatment fits my symptoms and other medicines? How will we monitor mood, sleep, appetite, and side effects? What should I do if suicidal thoughts appear?
