What should you know about mediopatellar plica syndrome?

Short answer
Mediopatellar plica syndrome is irritation of a tissue fold beside the kneecap, causing inner-knee pain, clicking, or catching during movement.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What it means
A plica is a flexible fold in the joint lining. The mediopatellar plica lies along the inner side of the knee and may become thickened or sensitive after repeated bending, kneeling, running, or a blow to the knee. Symptoms often appear when the knee moves through a particular angle rather than at rest.
People may feel soreness behind or beside the kneecap, a rubbing sensation, or a soft snap. Knee clicking can occur with several knee problems, so the sound alone does not establish the diagnosis. A clinician may examine tenderness, movement, strength, and signs of other causes such as cartilage or meniscus irritation.
What you can do
Early care commonly reduces activities that repeatedly provoke the pain while keeping the knee gently mobile. Ice wrapped in a cloth after activity, a gradual return to loading, and clinician-approved pain relief may help. Physiotherapy can address thigh and hip strength, movement control, and flexibility so the kneecap is loaded more evenly.
If symptoms continue despite rehabilitation, an orthopaedic clinician may discuss imaging or, in selected cases, knee arthroscopy. Arthroscopy uses small instruments to inspect the joint and may treat an irritated plica when the diagnosis is clear. The choice depends on examination findings, duration of symptoms, and response to non-surgical care.
When to seek care
Arrange an assessment if inner-knee pain, catching, or swelling keeps returning, limits walking or stairs, or does not settle after reducing the provoking activity. Seek urgent care after a major injury if you cannot bear weight, the knee changes shape, swelling appears quickly, or the joint becomes locked. Fever, marked redness, or a hot knee also needs prompt medical attention.
Questions for your doctor
Ask what findings support plica irritation, which activities should be modified, and how to progress strengthening. You can also ask whether another knee problem needs to be ruled out and what change would prompt a review of the plan.

Questions people ask
Can mediopatellar plica syndrome cause knee locking?
It can create a catching sensation, but true locking needs assessment because cartilage or meniscus problems can also restrict movement.
Is surgery always needed?
No. Many care plans begin with activity changes and physiotherapy; an orthopaedic clinician considers arthroscopy only when symptoms persist and the diagnosis is well supported.
