What it means

A plica is a thin fold in the joint lining. When the medial plica becomes irritated, it may rub against nearby structures as the knee bends and straightens. Pain is often felt along the inner edge of the kneecap or just above the joint line. Repeated squatting, stairs, running, cycling, or a sudden increase in training can aggravate it. Clicking or a catching sensation may accompany soreness, but these symptoms can also occur with other knee problems. A clinician uses your history and an examination to compare plica irritation with conditions such as kneecap pain or a meniscus problem.

What you can do

Temporarily reduce the movement that reliably provokes pain, while keeping gentle, comfortable motion. Ice after activity may ease irritation for some people; protect the skin and stop if it feels uncomfortable. A physiotherapist can guide exercises for thigh and hip strength, movement control, and a gradual return to sport. Do not repeatedly force a painful click or test the knee through sharp pain. If symptoms continue despite sensible activity modification, arrange an assessment so the diagnosis and rehabilitation plan can be reviewed. Physiotherapy is commonly considered before an operation; knee arthroscopy may be discussed when persistent symptoms have not improved with non-surgical care.

When to seek care

Seek prompt medical advice if the knee becomes locked, gives way repeatedly, swells markedly after an injury, or you cannot bear weight. Urgent assessment is also sensible for a hot, red knee with fever, or severe pain after a major impact. Arrange a routine appointment for inner-knee pain that keeps returning, limits stairs or walking, or does not settle after reducing aggravating activity. These features do not identify one diagnosis by themselves, so an examination matters.
