Measles rash in the mouth

Measles can cause tiny pale spots on a red background inside the cheeks. They may be difficult to see and are not the same as ulcers on the lips or a coating on the tongue. Mouth findings are considered together with fever, cough, runny nose, sore eyes, swollen glands and the later skin rash.

Mouth spots have other possible causes, so their presence does not confirm measles. Avoid touching the area repeatedly, as this can cause irritation and make observation harder.

Note when the mouth changes appeared and whether eating, drinking or swallowing is uncomfortable. Offer frequent fluids and soft foods if needed. Do not share cups, cutlery or toothbrushes. If measles is possible, call a GP or health service before travelling to the surgery and describe the mouth findings and any rash.

Keep the person resting and record fever, urine output and alertness. Do not use mouth medicines unless they are suitable for the person and recommended by a pharmacist or clinician.

Get urgent help if the person cannot swallow fluids, has breathing difficulty, becomes confused or unusually sleepy, has a seizure, or shows severe dehydration. Prompt assessment is also needed for a rapidly spreading rash, persistent fever, or illness in someone who is very young, pregnant or immunocompromised.

Ask whether the mouth spots fit the wider illness, whether examination or testing is needed, how to protect household contacts, and what food or drinks may ease soreness. Provide the vaccination record and the timeline for fever, mouth changes and skin rash.
