What does spotting during pregnancy mean?

Spotting may appear as pink, red, or brown marks on underwear or toilet paper. It can occur when the cervix is more sensitive, including after sex or an examination. Infection, changes near the cervix, and problems involving the placenta or pregnancy can also cause bleeding. A symptom does not identify the cause by itself; the amount, colour, timing, pain, and pregnancy stage all help a clinician assess it.

Bleeding between periods is a separate symptom when someone is not pregnant, but the same description can be used for light bleeding at other times. Conditions such as cervical dysplasia, fibroids, polyps, or chlamydia may be considered when the clinical context fits. Cervical cancer and womb cancer are less common explanations, but unusual bleeding still deserves medical review.

Contact your maternity team or obstetric clinician for advice, even if the spotting is light. Note when it began, how much you see, its colour, whether clots are present, and whether you have cramps, pelvic pain, fever, dizziness, or fluid leakage. Use a pad so the amount can be observed; avoid putting anything in the vagina until you have received advice. Do not drive yourself if you feel faint.

Seek urgent assessment for heavy or increasing bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, fever, or feeling suddenly unwell. Contact maternity services promptly for reduced fetal movement later in pregnancy, fluid leakage, or bleeding after a recent injury. These warning signs need assessment because bleeding can sometimes accompany a pregnancy complication.

Ask what the bleeding pattern could represent at your stage of pregnancy, whether you need an examination or ultrasound, and which symptoms should bring you back immediately. Ask whether your blood group affects the next steps and when normal activities, sex, or exercise can be resumed.
