A person may become confused, unusually sleepy, have trouble speaking, lose consciousness, or develop seizures. These changes are emergencies, especially after travel or residence in a malaria-risk area.

What it means

Malaria is an infection carried by certain mosquitoes. In severe illness, infected red blood cells and the body's inflammatory response can interfere with the brain and other organs. Fever, chills, and muscle aches can occur with malaria, but brain symptoms signal a more serious presentation. The symptom pattern cannot confirm the diagnosis; clinicians use an examination and testing, often including a blood test.

What to do now

If confusion, a seizure, fainting, or marked drowsiness is present, arrange emergency help immediately. Keep the person on their side if unconscious or having a seizure, clear nearby objects, and do not put anything in their mouth. Tell the medical team about recent travel, mosquito exposure, medicines, and when the fever began. Do not drive someone who is confused or fitting.

When to seek care

Seek urgent care for any fever after travel to a malaria-risk area, particularly when it comes with confusion, breathing difficulty, repeated vomiting, severe weakness, jaundice, a seizure, or inability to stay awake. Malaria can worsen quickly, and a normal temperature at one moment does not exclude it. Emergency clinicians can begin assessment while arranging tests and treatment.
