Painless Lump Under the Skin of the Penile Shaft

The penile shaft contains skin, hair follicles, glands, blood vessels, and connective tissue. A blocked follicle, cyst, scar, or other benign change may feel like a small lump. Infection is more concerning when there is warmth, redness, drainage, tenderness, fever, or rapidly increasing swelling. Blisters or a burning rash may fit a different pattern, including shingles, while recurring lumps in nearby skin folds can occur with hidradenitis suppurativa. Appearance and sexual history help guide assessment, and a painless lump should not be self-diagnosed from a photograph.

Do not squeeze, cut, puncture, or apply unprescribed creams to the lump. Wash gently, keep the area dry, and avoid friction until you know what it is. Arrange a visit with a dermatologist, sexual-health clinician, or urologist if it remains or you are worried. The clinician may inspect the skin, ask about new products or sexual contact, and decide whether testing or treatment is needed. Treatment depends on the cause; antibiotics, antiviral treatment, or abscess drainage are not interchangeable choices.

Seek prompt care for pain that develops, spreading redness, pus, fever, open sores, blisters, discharge, or swollen groin glands. Get urgent help if swelling makes urination difficult, the foreskin becomes trapped, or the penis changes colour. Avoid sexual contact if there are sores, drainage, or a possible infection until a clinician gives guidance.
