Lower thumb palm pain: what to know

Short answer
Lower thumb palm pain often reflects irritation around the thumb joint or tendons and should be assessed if function or symptoms keep worsening.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Often aggravating
- Pinching, gripping, jar opening and repetitive thumb use
- Protective option
- A correctly fitted thumb or wrist support during difficult tasks
What it may mean
Pain at the base of the thumb can follow repeated gripping, texting, tool use, lifting or a fall. It may be felt in the palm, wrist or thumb side of the hand and can worsen when opening jars, turning keys or pinching. De Quervain's tenosynovitis affects tendons on the thumb side of the wrist and may cause pain with thumb movement or wrist loading. A joint sprain, arthritis or nerve irritation can produce a similar area of discomfort, so the trigger and examination are useful.
What you can do now
Reduce forceful pinching, prolonged gripping and repetitive thumb movements for a short period. Keep the thumb gently mobile within comfort, but do not repeatedly test painful movements. A supportive thumb or wrist splint may help during aggravating tasks; it should not make the fingers numb or discoloured. Cold can soothe a recent flare, while warmth may help stiffness. A clinician or hand therapist can check tendon loading, joint stability and grip technique if daily tasks remain difficult.
When to seek care
Seek urgent assessment after a fall or crush injury if the thumb looks deformed, becomes very swollen, feels numb or cannot move. Arrange a routine review for persistent pain, increasing weakness, clicking, reduced grip or symptoms that interfere with work and sleep. Sudden colour change or a cold hand also needs prompt medical attention.
Questions for your doctor
Is the pain coming from a tendon, joint or ligament? Would a splint help and for which activities? Which exercises are safe? Do I need an examination or imaging? What changes should prompt faster review?

Questions people ask
Why does gripping make thumb-base pain worse?
Gripping loads the thumb tendons and joints, so irritated tissues may hurt more during pinching or forceful hand use.
Should I keep my thumb completely still?
Short-term support can help, but gentle comfortable movement usually prevents unnecessary stiffness unless a clinician advises immobilisation.
