Lower back right-side muscle pain

Short answer
Lower back right-side muscle pain may come from a strain, but location alone cannot confirm a muscle cause.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Notice whether bending, walking, coughing, or pressing the area changes the pain.

- Leg weakness, groin numbness, urinary changes, fever, or vomiting require prompt attention.
What right-sided pain can indicate
Pain on the right may follow lifting, sport, prolonged posture, or a sudden twist, especially when it is tender and changes with movement. The facet joints, sacroiliac joint, or connective tissues can create a similar one-sided ache. A herniated disc may irritate a nerve and send pain into the buttock or leg, sometimes with tingling or weakness. Lumbar spondylosis and non-specific low back pain are other possible explanations.
Pain deeper in the side or back with fever, nausea, urinary changes, or feeling unwell may not be a simple muscle problem. Note what triggers the pain and whether it spreads.
What to do
Avoid the lift, twist, or exercise that started the pain, but keep up gentle walking if comfortable. Try heat or a wrapped cold pack, adjust sitting and sleeping positions, and return to normal activity in small steps. A clinician or pharmacist can advise on pain medicine based on your medical history. Arrange a review if the pattern is unclear or not settling.
When to seek care
Seek urgent care for severe or rapidly worsening pain, fever, vomiting, blood in the urine, inability to pass urine, new leg weakness, numbness around the groin, or bladder or bowel control changes. Prompt assessment is also appropriate after a significant injury or when pain is accompanied by unexplained weight loss or feeling very unwell.
Questions for your doctor
Ask: Does movement or tenderness suggest a muscle, joint, or nerve source? Could urinary or abdominal symptoms point to another cause? Do I need an examination or imaging? Which activities and pain-relief options are safe while I recover?

Questions people ask
Does right-sided lower back pain always mean a pulled muscle?
No. Muscles, joints, nerves, and nearby organs can all cause pain in that area.
Can I exercise with right-sided back pain?
Gentle walking may be reasonable if tolerated, but stop movements that sharply increase pain and seek advice if symptoms persist.
