Lower back pain and spotting between periods

Spotting between periods can occur for several reasons, including hormonal changes, contraception-related bleeding, infection, or pregnancy-related problems. Lower back pain may be connected to the same pelvic process, or it may be a separate issue such as muscle strain, sacroiliac joint dysfunction, or non-specific low back pain. The combination does not establish one diagnosis.

Write down when the spotting began, its colour and amount, cycle timing, pain location, and any pelvic pressure, discharge, fever, urinary change, or dizziness. Mention contraception and the possibility of pregnancy during an assessment.

Arrange a medical appointment for new or repeated spotting with back pain. Until reviewed, use a pad if helpful for tracking, avoid activities that clearly aggravate the back, and keep hydrated. Do not ignore a missed period or pregnancy possibility; tell the clinician because it changes the urgency and assessment.

Seek urgent care for severe or one-sided pain, fainting, marked dizziness, shoulder-tip pain, fever, heavy bleeding, or a positive pregnancy test with pain or bleeding. Get prompt review for worsening pain, foul-smelling discharge, pain when urinating, new weakness, or numbness around the groin.

Ask: What could explain the spotting and back pain together? Should pregnancy, infection, pelvic causes, or a back condition be checked? What changes in bleeding or pain require urgent care? Which pain-relief options are appropriate if pregnancy is possible?
