Lower back pain during intercourse

Back extension, twisting, or sustained pressure can aggravate a sensitive lower back. Muscle strain, facet joint syndrome, sacroiliac joint dysfunction, and non-specific low back pain are possible mechanical causes. Pain deep in the pelvis, pain with penetration, bleeding, discharge, urinary symptoms, or cycle-related symptoms may point toward a pelvic or urinary issue instead. Pain travelling down a leg, tingling, or weakness suggests nerve irritation and should be described clearly to a clinician. The timing, position, depth, and whether pain continues afterward are useful details.

Stop when pain becomes sharp or escalating, and communicate about pressure, depth, and position. Try positions that keep the lower back supported and neutral, use pillows for comfort, and allow a slower pace. Gentle walking and heat may help afterward if they feel comfortable. Do not push through radiating pain or numbness. If pain keeps returning, a clinician or physiotherapist can assess movement, spinal and pelvic contributors, and suitable exercises. Seek sexual-health or gynaecological advice when pelvic symptoms are present.

Seek urgent care for severe sudden pain, fainting, heavy bleeding, fever, or vomiting. New leg weakness, numbness around the genitals, or loss of bladder or bowel control is an emergency. Arrange an appointment for persistent pain, bleeding after intercourse, unusual discharge, urinary burning, or pain that limits intimacy or daily movement.
