Lower back ache when lying down: what to know

Short answer
Lower back ache when lying down may reflect position or stiffness, but night pain that persists needs assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why lying down can hurt
A mattress, pillow position, prolonged stillness, or turning in bed can load the lower back differently from standing. Muscles may tighten after a day of activity, and joint or disc irritation can become more noticeable when distractions stop. [Lumbar Spondylosis](/conditions/lumbar-spondylosis/) is one possible back-related cause, but lying-down pain does not identify it. Note whether changing position helps, whether pain wakes you, and whether symptoms travel into a leg.
Make rest more comfortable
Try side-lying with a pillow between the knees, or place a pillow under the knees when lying on your back. Roll onto your side before getting out of bed, then rise using your arms. Gentle movement during the day can limit stiffness; avoid forceful stretching at night. Check that the sleeping surface is supportive and that your usual position is not twisting the hips. Record sleep disruption and daytime triggers for a clinician or physiotherapist.
When to arrange care
Seek urgent help for new bladder or bowel difficulty, groin numbness, significant weakness, fever, serious injury, or rapidly escalating pain. Arrange a prompt appointment if pain repeatedly wakes you, is present at rest, continues despite position changes, or comes with unexplained illness or leg symptoms.
Questions for an appointment
Ask which sleeping positions are suitable, whether your examination suggests a muscle, joint, nerve, or hip source, and whether you need a tailored exercise plan or further assessment.
