Short answer

Lower back ache during a period often accompanies cramping, but severe or unusual pain should be assessed.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Warmth and gentle movement can reduce guarding and stiffness

Why it can happen

During menstruation, uterine contractions can produce cramping that is felt in the lower back, pelvis, or thighs. Poor sleep, reduced activity, and muscle guarding may add stiffness. Pain that is new, progressively stronger, present outside the period, or associated with pain during sex, unusual discharge, or heavy bleeding may need a broader pelvic assessment. A separate spinal condition such as [Lumbar Spondylosis](/conditions/lumbar-spondylosis/) can also cause back pain, so the menstrual timing is useful information rather than a diagnosis.

Ways to ease it

Use a warm pack on the lower abdomen or back, take gentle walks, and try comfortable hip movements rather than staying still for long periods. Follow the label for any over-the-counter pain medicine and check with a pharmacist or clinician if you have ulcers, kidney disease, take blood thinners, or could be pregnant. Record cycle dates, bleeding changes, pain location, and how daily activities are affected.

Warning signs

Seek urgent care for fainting, sudden intense pain, fever, repeated vomiting, heavy bleeding, or severe pain when pregnancy is possible. Book an appointment when pain repeatedly disrupts work or sleep, does not respond to usual measures, begins between periods, or changes from your normal pattern.

Useful questions

Ask whether the pattern fits ordinary menstrual cramps, whether another pelvic or back cause should be considered, which medicines are safe for you, and whether an examination or further tests are warranted.
