Lower abdominal pain during sex: could you be pregnant?

Pain during penetration may relate to dryness, muscle tightening, cervical contact, infection, an ovarian cyst, endometriosis, adenomyosis, or fibroids. Pelvic inflammatory disease can cause deep pain, discharge, fever, or bleeding. Sex-related pain is not a reliable way to tell whether conception occurred. If pregnancy is possible, a pregnancy test at the appropriate time is more useful; pain with bleeding or one-sided pain needs medical attention because ectopic pregnancy is a concern.

Stop or change sexual activity when pain begins, and do not push through sharp or deep pain. Note whether pain is at entry or deep in the pelvis, its side and duration, cycle timing, contraception, and any bleeding or discharge. Arrange a clinician visit for new, repeated, or persistent pain. They may discuss pregnancy testing, infection testing, examination, or ultrasound.

Get urgent help for severe or one-sided pain, faintness, shoulder-tip pain, heavy bleeding, fever, or repeated vomiting, especially after a missed period or with a positive pregnancy test. These symptoms can signal an ectopic pregnancy or another urgent pelvic problem.
