Lower abdominal pain and spotting in early pregnancy: what should you know?

Light spotting can occur for different reasons, including changes around the cervix, but it cannot be interpreted safely without considering pain, pregnancy timing, bleeding amount, and other symptoms. Mild cramps may also occur as the body changes. These symptoms do not by themselves confirm that a pregnancy is progressing normally or indicate a particular diagnosis.

An ectopic pregnancy can present with lower abdominal or pelvic pain and bleeding, sometimes more strongly on one side. Ovarian cysts and other pelvic conditions can also cause pain. Do not use symptom intensity alone to rule out a serious problem.

Contact a pregnancy or urgent-care service for advice, especially if the pregnancy has not yet been confirmed inside the uterus. Note the colour and amount of bleeding, pain location and severity, dizziness, shoulder-tip pain, fever, and any tissue passed. Use pads rather than relying on memory to judge whether bleeding is increasing.

Avoid strenuous activity until you have received advice if pain or bleeding is continuing. Do not start or stop medicines without checking with a clinician or pharmacist; tell them about the pregnancy and any previous ectopic pregnancy or pelvic surgery.

Go to emergency care now for severe or worsening pain, one-sided pain with dizziness, fainting, shoulder-tip pain, heavy bleeding, pale or clammy skin, marked weakness, or collapse. These warning signs can occur with internal bleeding and require immediate assessment.
