What it may mean

A muscle strain can happen when abdominal or nearby hip muscles are stretched beyond their usual load. Pain may be local, tender when touched, or worse when you twist, sit up, cough, laugh, or lift. Tightness and guarding are common. Pain after exercise can also reflect delayed muscle soreness, while pain that spreads to the back, groin, or thigh may need a broader examination. Lower abdominal pain is not automatically muscular: bowel, urinary, reproductive, or abdominal-wall problems can feel similar.

A clinician may ask when the pain began, what movement triggered it, whether there is swelling or a bulge, and whether bowel or bladder symptoms are present.

What you can do now

Pause the movement that provokes pain and keep gentle, comfortable activity rather than prolonged bed rest. For the first day, a wrapped cool pack may help; later, warmth can feel soothing if there is no swelling. Avoid heavy lifting, forceful abdominal exercises, and deep stretching until ordinary movements are easier. Return gradually, beginning with walking and light tasks, then building load without sharp pain. A physiotherapist can check trunk and hip movement, guide strengthening, and help identify technique or workload changes.

When to seek care

Seek urgent care for severe or rapidly increasing pain, a rigid or very tender abdomen, fainting, repeated vomiting, blood in stool or urine, fever, or inability to pass urine or stool. Get prompt assessment for a new painful bulge, pain after a significant injury, or pain with testicular symptoms or unusual vaginal bleeding. Arrange a clinical review when pain limits walking, keeps returning, or does not begin improving with relative rest.
