What should you know about low temperature and rash?

Low temperature can dry or irritate the skin and may trigger an itchy, swollen, or color-changing rash in some people. Cold air, wind, and indoor heating can reduce moisture at the skin surface. A rash that appears after cold exposure can also have another cause, such as an irritant, infection, medication reaction, or an inflammatory skin condition. The timing, appearance, location, and other symptoms help a clinician assess it.

Dryness from cold weather may cause tightness, scaling, cracking, or stinging, especially on the hands, face, and lips. Cold-triggered hives can produce raised, itchy areas soon after exposure; swelling may occur where the skin became cold. A rash that remains after warming up, spreads, becomes scaly, or returns in different settings deserves a broader assessment. Similar-looking rashes can occur with Atopic Dermatitis, Contact Dermatitis, Drug Rash, Heat Rash, Intertrigo, Psoriasis, Ringworm, or Scabies, so appearance alone cannot establish the cause.

Warm the skin gradually and protect it from wind with suitable clothing. Use lukewarm rather than hot water, a gentle fragrance-free cleanser, and a thick moisturizer soon after washing. Avoid scratching, harsh scrubs, and newly introduced creams until the pattern is clearer. Note when the rash started, what was cold, whether it improves with warmth, and any new products or medicines. Take a photograph during a flare if the skin changes before an appointment.

Seek urgent medical help for trouble breathing, swelling of the lips or tongue, faintness, confusion, or a rapidly worsening reaction after cold exposure. Arrange prompt assessment for blistering, severe pain, skin that becomes numb or unusually pale, blue, or dark, or a rash with fever or marked illness. Contact a clinician if the rash keeps returning, lasts despite avoiding cold and caring for dry skin, or affects the eyes, mouth, or genitals.
