What should you know about low psa metastatic prostate cancer?

PSA is made by many prostate cells, but metastatic tumours can change their behaviour and produce less PSA than expected. A low result therefore cannot, on its own, rule out active or spreading disease. Doctors may review imaging, biopsy findings, alkaline phosphatase or other blood tests, and the pattern of symptoms. Spread can involve bones or other organs, and treatment decisions depend on the overall picture rather than one measurement.

Keep a record of new pain, weakness, urinary changes, weight change, and treatment effects, and take it to appointments. Ask whether repeat imaging or a biopsy is needed and whether tumour profiling could identify a treatment option. Depending on the location and biology of disease, care may include hormone-based treatment, chemotherapy, radiotherapy, targeted radioisotope therapy, or bone-strengthening therapy. Local treatments such as brachytherapy or prostate removal surgery are considered only when they fit the disease pattern.

Get emergency help for new leg weakness, numbness around the groin, loss of bladder or bowel control, or severe back pain, because these can signal pressure on the spinal cord. Seek urgent advice for inability to pass urine, fever during treatment, sudden confusion, or rapidly worsening bone pain. Difficulty passing urine needs assessment even when PSA is low.

Ask what evidence confirms spread, which sites need imaging, and whether the tumour’s appearance suggests a less PSA-producing pattern. Discuss the goals of treatment, how response will be monitored when PSA is an imperfect marker, and what symptoms should trigger urgent contact. Ask about bone health, pain control, and support for daily activities.
