What should you know about looks like ringworm?

Ringworm is a superficial fungal infection that often creates an expanding, scaly edge with clearer skin toward the centre. Eczema, contact dermatitis, psoriasis, granuloma annulare, and other rashes can produce a similar outline. A ring does not prove fungal infection, and the appearance can change after creams have been used.

A clinician considers scale, border, itch, number of patches, body site, exposure to people or animals, and whether the rash affects nails or scalp. If the diagnosis is uncertain, a skin scraping or other examination may help select treatment.

Keep the area clean and dry, avoid sharing towels or clothing, and wash items that touch the patch regularly. Avoid scratching and avoid combination creams unless a clinician recommends them, since an unneeded steroid may alter the appearance or allow a fungal infection to spread. If ringworm is confirmed, use the prescribed topical antifungal for the advised course and follow instructions even when the edge begins to fade. Extensive, recurrent, scalp, nail, or treatment-resistant infection may need oral therapy.

Arrange an assessment if the rash is spreading, painful, oozing, very inflamed, or not improving with appropriate care. Seek prompt review for scalp involvement, hair loss, nail changes, facial or eye-area lesions, or infection in someone with reduced immunity. Urgent help is appropriate for fever, rapidly worsening swelling, or severe pain.

Ask whether the border and scale fit ringworm, whether testing is needed, and which topical antifungal is suitable. Ask when oral antifungal treatment is considered, how to reduce spread at home, and when lack of improvement should prompt reassessment.
