What should you know about locally advanced prostate cancer?

The cancer may involve tissue beside the prostate, the seminal vesicles, or nearby lymph nodes without being described as distant spread. Tests may include examination, prostate-specific blood testing, scans and tissue review. The results help the team understand the extent, grade and biology of the disease.

Some people have urinary symptoms, while others have no noticeable change. Difficulty passing urine can occur with prostate conditions, but it does not by itself establish cancer or its stage. Your plan depends on scan findings, overall health, symptoms and personal priorities.

Ask the urology and oncology teams how the disease is staged and what each treatment aims to achieve. Options may include radiotherapy with hormone treatment, surgery in selected situations, or a combination of approaches. Brachytherapy places radiation near the prostate; radiotherapy can treat the prostate region and nearby targets.

If the disease later affects bones, bone-strengthening therapy or targeted radioisotope therapy may be discussed. Prostate removal surgery and robotic cancer surgery are separate topics to review when an operation is being considered. Keep a record of urinary, bowel, sexual and energy changes during care.

Contact your care team promptly for inability to pass urine, worsening blood in the urine, fever with urinary symptoms, or severe new pelvic or back pain. New leg weakness, numbness, or loss of bladder or bowel control needs emergency assessment because pressure on nerves can cause lasting harm.

Ask where the cancer has spread locally, whether nearby lymph nodes are involved, and what the staging tests show. Discuss the expected effects of each option on urine, bowel function, erections and fertility, plus how symptoms will be managed.
