Linear morphea on the face: what to know

Morphea, also called localised scleroderma, causes areas of skin to become tighter, firmer or differently coloured. A linear facial form may appear as a band or groove on the forehead or scalp and can extend across nearby tissues. It is not the same as systemic scleroderma, but its location makes careful examination particularly useful.

A clinician may check skin texture, movement, facial symmetry, hair changes and nearby joints. Children and adolescents may need monitoring while the face is still growing. The active phase can be subtle, while later changes may reflect established tissue alteration rather than ongoing inflammation.

Treatment depends on activity, depth and location. A dermatologist may use anti-inflammatory medicines, topical calcineurin inhibitors for selected areas, or light-based treatment such as narrowband UVB when appropriate. Treatment plans are individual, and the goal may be to quiet active disease, protect function and monitor change rather than alter every established mark.

Seek a prompt review for a new or expanding groove, increasing tightness, pain, weakness of facial movement, vision changes, severe headache, seizures, or a new difference in facial growth. Urgent assessment is appropriate for sudden weakness, sudden vision loss or severe neurological symptoms.

Ask whether the area appears active, whether deeper tissues need imaging or another assessment, and which treatment is suitable for the face. Clarify how often reviews are needed, what changes should be photographed, and whether an eye, dental or neurological assessment is relevant.
