Linear lichen planus treatment: what to know

Lichen planus is an inflammatory skin condition that may produce small, firm, itchy bumps or flat-topped patches. In the linear form, lesions follow a line or scratch-like pattern, sometimes along the path of minor skin injury. The appearance can be mistaken for a simple rash, so a dermatologist may examine the pattern closely and occasionally recommend a skin sample.

Colour changes can remain after the active inflammation settles, especially on darker skin tones. Those marks do not necessarily mean the condition is still spreading. Scratching can irritate nearby skin and make symptoms harder to settle; gentle skin care and short nails can help reduce damage.

A dermatologist may prescribe a topical corticosteroid for active, limited areas. The strength, amount and duration should match the body site, because facial and thin-skin areas need different precautions from thicker skin. If the pattern is extensive, persistent or difficult to control, an oral corticosteroid or another anti-inflammatory approach may be considered.

Use fragrance-free moisturiser, avoid picking, and protect affected skin from friction. Arrange a dermatology review if a line of bumps is spreading, repeatedly returning, very itchy, painful, crusted or changing in colour. Seek prompt care for blistering, pus, fever, rapidly worsening redness, or involvement of the eyes, mouth or genitals. These features can indicate a different problem or a complication that needs direct examination.

Ask whether the linear pattern needs a biopsy, which areas should receive treatment, how long to use the prescribed medicine, and what colour change may remain after inflammation improves. Also ask how to recognise a flare and whether any current medicines or supplements could be contributing.
