Light Brown Spotting in the First Trimester

Brown discharge can appear when blood takes longer to leave the vagina and changes colour. It may occur after sex, a pelvic examination, or from the normal sensitivity of the cervix in early pregnancy. The colour alone cannot identify the cause or confirm that a pregnancy is progressing normally.

A small amount of spotting may settle without treatment, but possible explanations also include irritation of the cervix, an infection, a threatened miscarriage, or an ectopic pregnancy. An ectopic pregnancy develops outside the uterus and can cause one-sided pain, shoulder-tip pain, dizziness, or fainting. These symptoms need urgent medical assessment.

Tell the clinician whether the spotting is increasing, whether it contains clots or tissue, and whether you have cramps, pelvic pain, fever, unusual discharge, or pain when passing urine. Conditions such as Chlamydia can cause abnormal bleeding, while Uterine Fibroids or Endometrial Polyps may also contribute to bleeding outside a usual cycle.

Contact your maternity service, obstetrician, gynaecologist, or another qualified clinician for advice, even if the spotting is light and you feel well. Use a pad rather than a tampon so you can observe the amount and colour. Note when it started, how often you see it, and any triggers such as sex or an examination.

A clinician may ask about your pregnancy dates and symptoms and may arrange an examination, ultrasound, or blood tests depending on your situation. Do not start medication or stop prescribed treatment without checking what is suitable in pregnancy.

Seek urgent medical help if bleeding becomes heavy, you develop severe or worsening abdominal or pelvic pain, pain mainly on one side, shoulder-tip pain, faintness, marked dizziness, fever, or feel very unwell. Passing clots or tissue also warrants prompt assessment.

Do not wait for a routine appointment when these warning signs occur. If you are fainting, have severe pain, or cannot reach your maternity team promptly, use local emergency services.

You could ask: What examinations or tests do I need? Could the spotting be related to my cervix, an infection, or the location of the pregnancy? Which symptoms should make me seek emergency care? Should I avoid sex, exercise, or anything else while I am being assessed?
