Stage 4 means the cancer has spread beyond the original lung or to a distant part of the body. The extent and location of spread, the cancer’s subtype, genetic changes, breathing reserve, other medical conditions, and day-to-day strength all influence treatment choices and outlook.

Testing may include tissue examination, scans, and molecular or biomarker assessment. These results can show whether a targeted treatment or immunotherapy is appropriate. Doctors also consider symptom burden and what matters most to you when discussing goals.

Ask for a clear explanation of the treatment goal, how response will be checked, and what side effects should be reported. Depending on the findings, care may involve immunotherapy, targeted therapy, chemotherapy, radiotherapy, or a combination. A focused procedure such as stereotactic body radiotherapy may be considered for a selected site, while surgery has a role only in carefully selected situations.
