Lichenoid Cheilitis: What to Know

The lower lip is often the most noticeable site, with scaling, cracking, a pale or red border, or a patch that feels rough. Burning may increase with toothpaste, spicy food, citrus, wind, or lip products. A lichenoid pattern can be triggered by contact with an ingredient, irritation, dental material, or a medicine, but the cause is not always obvious. Cheilitis has several forms, so a clinician may need to inspect the lip closely and review exposures.

Apply a bland, fragrance-free emollient regularly, especially after eating and before sleep. Avoid licking, biting, peeling scale, fragranced balms, and products that sting. Change only one suspected product at a time so improvement is easier to recognise. A clinician may prescribe a short course of topical corticosteroids when inflammation is active; use the medicine only on the advised area and for the advised duration. If a dental material or medicine is suspected, discuss alternatives with the relevant professional rather than stopping treatment yourself.

Arrange an examination if a rough, crusted, sore, or discoloured lip area persists despite gentle care, repeatedly cracks or bleeds, becomes thickened, or develops a lump. Prompt review is especially useful for a change limited to one spot. Seek urgent help for rapidly increasing swelling, trouble breathing, or difficulty swallowing. A clinician may take a swab or small biopsy when the appearance does not clearly identify the cause.
