Lichen Planus Under the Tongue: What to Know

The underside of the tongue may show a lacy white pattern, smooth red areas, or shallow tender sores. Symptoms can come and go, and some people notice little discomfort while others feel burning with citrus, chilli, or hot drinks. A clinician may examine the tongue, cheeks, and gums because oral lichen planus can affect more than one mouth area. A similar-looking patch can have other explanations, so appearance alone does not confirm the diagnosis.

Use a soft toothbrush and avoid tobacco, alcohol-based mouthwash, and foods that sting the sore area. Keep a note of new medicines, dental products, or fillings introduced before symptoms began. A dentist, oral medicine clinician, or dermatologist may recommend a mouth-safe topical corticosteroid for inflamed areas; treatment should be used exactly as directed. If symptoms are widespread or difficult to control, oral corticosteroid treatment may be discussed. Do not stop prescribed medicine without guidance.

Arrange a prompt assessment for a mouth patch or sore that does not settle, repeatedly bleeds, becomes firmer, or changes in colour or texture. Seek urgent help if swelling makes breathing or swallowing difficult, or if you cannot keep fluids down. An examination may include checking nearby tissues and, when the diagnosis is uncertain, taking a small sample for laboratory assessment.
