Lichen planus on the scalp: what pictures can show

Scalp lichen planus can cause redness, scaling, crusting, or soreness around follicles. It may be itchy, and affected areas can develop reduced hair growth or smooth patches where inflammation has damaged follicles. Similar appearances can come from eczema, psoriasis, infection, or irritation from hair products. A dermatologist may use close inspection, dermoscopy, or a small biopsy to distinguish these causes.

Wash gently, avoid scratching, and pause fragranced dyes, relaxers, or products that sting the area. Do not pick scale or pull at affected hairs. Keep photographs in consistent lighting to show change at appointments, but do not use them to self-diagnose. A clinician may prescribe a topical corticosteroid for scalp inflammation; oral corticosteroid treatment may be considered when disease is more widespread or difficult to control.

Book a dermatology assessment for persistent scalp scale, follicle-centered bumps, tenderness, crusting, or a patch of hair thinning. Seek earlier review if hair loss is progressing, the scalp is painful, or there is drainage or marked swelling. Prompt diagnosis can help protect remaining follicles.
