Lichen planus of the esophagus

The esophagus can become inflamed and narrowed, so food may feel slow to pass or stick behind the breastbone. Some people also notice pain when swallowing, reflux-like discomfort or changes in their established mouth or skin symptoms. These symptoms are not specific to lichen planus. A gastroenterology assessment may include an upper endoscopy, during which the lining is viewed and tissue samples can be taken to distinguish inflammation from other causes of narrowing.

Do not try to treat swallowing difficulty with leftover medicines. Arrange medical review so the cause and severity can be assessed. Treatment may involve a medicine applied to affected mucosa or an oral corticosteroid, depending on findings and the rest of the condition. A specialist may also address a narrowing, review medicines that can irritate the esophagus, and coordinate care with a dermatologist or dentist when skin or mouth lesions are present. Take small bites and chew thoroughly until you have been assessed.

Seek urgent care if food becomes stuck, you cannot swallow liquids or saliva, breathing is affected, or swallowing pain is severe. Arrange prompt review for progressive difficulty swallowing, repeated choking, vomiting or unexplained weight loss. These warning signs reflect a need to protect nutrition and the airway, not proof of a particular diagnosis.

Ask which tests are appropriate, whether a biopsy is needed, how inflammation or narrowing will be treated, and which symptoms should trigger urgent help. Tell the clinician about mouth soreness, skin lesions, reflux symptoms, swallowing changes and every medicine or supplement you use.
