Lichen Planus Without Itching

Some lichen planus lesions are noticed because of their colour, shape, or texture rather than discomfort. Skin changes may be flat-topped and purple or reddish, with fine white lines on the surface. Oral disease may produce a lacy white pattern or soreness, while nail or scalp involvement can show structural changes. Absence of itch does not confirm or exclude lichen planus, and another rash can look similar.

Photograph the area in good light and record when it appeared, whether it is spreading, and any changes in the mouth, nails, hair, or genital skin. Use gentle, fragrance-free products and avoid picking or self-treating with strong creams. A dermatologist can assess the pattern and decide whether observation, a topical corticosteroid, or another approach is suitable.

Arrange a review for an unexplained rash that persists, changes, or affects a sensitive site even when it does not itch. Seek prompt care for mouth ulcers, difficulty swallowing, repeated bleeding, painful genital lesions, or rapid change in a nail or scalp area. Urgency comes from these warning features, not from whether itching is present.
