Letrozole and water retention

Letrozole lowers oestrogen production. Changes in hormone levels can occur alongside symptoms such as ankle or hand swelling, tight shoes, a feeling of bloating, or a change on the scales. Swelling can also have other causes, including a heart, kidney, liver, vein, medication, or clot-related problem, so the symptom alone does not identify the cause.

Notice where the swelling is, whether it affects one side or both, and whether it leaves a temporary dent after gentle pressure. Record when it began and any breathlessness, pain, redness, or rapid change.

Keep taking letrozole as prescribed until the clinician managing your treatment advises otherwise. Contact that team about persistent swelling, a new change in weight, or discomfort that affects walking or daily activities. They may review other medicines, examine the swollen area, and decide whether tests are needed.

Avoid starting water tablets, herbal products, or a strict low-salt plan without advice. If your clinician agrees, gentle movement and raising swollen legs while resting may ease mild dependent swelling.

Seek urgent medical help for sudden swelling in one leg or arm, especially with warmth or pain; new chest pain; severe breathlessness; fainting; confusion; or swelling of the lips, tongue, or throat. These symptoms can signal a serious problem and need immediate assessment.

Ask whether the pattern of swelling could relate to letrozole or another cause, what changes should be recorded at home, and which symptoms should prompt same-day contact. Also ask before using a diuretic or changing your dose.
